TERMS & CONDITIONS

TREATMENT

  • All treatments are by appointment only.

  • All clients must be willing to sign a consultation form during initial appointment.

  • Treatment will only be conducted if I deem the treatment to be safe for the client to receive.

  • GP/Consultant permission may be required before treatment can commence.  If you are unsure as to whether you may require GP's permission please discuss this with me prior to booking an appointment.

  • The client may be asked to remove some items of clothes to facilitate treatment.

  • All clients age 16 years and under MUST be accompanied by a parent or legal guardian.

CONSENT TO TREATMENT POLICY

I consent to treatment as outlined to me. Due to the physical nature of Remedial Soft Tissue Therapy treatment, various parts of the body may require physical contact including, but not limited to, the jaw, throat, chest, abdomen, hip, thigh and buttock regions. If at any stage you are uncomfortable with any of these procedures please inform me. Post treatment local soreness and/or minor bruising, whilst avoided, is always possible and considered a normal and acceptable potential side effect of Remedial Soft Tissue Therapy/Sports Massage. Overall benefits from treatment should be felt regardless. If not, I will review your history & treatment programme or refer you as necessary. I cannot be held responsible for any diagnosed, undiagnosed, unconfirmed or suspected underlying conditions or your failure to disclose them. Nor any conditions that are, as yet, not known about by either party and do not show up in standard history taking or assessments as performed at this level of clinical practice.
I declare that I am fully capable of making the decision to receive Remedial Soft Tissue Therapy treatment and I consent to this in full knowledge of what is involved.

CANCELLATION/LATE ARRIVALS POLICY

  • If you need to change or cancel your appointment please provide at least 24 hours notice before your appointment time.  Unfortunately missed appointments could incur a fee.  Thank you for your understanding and co-operation.

  • I value your time, and in the unlikely event that I need to cancel your appointment, for example if the room or environment is unsuitable for the treatment to be provided or if I am unwell or injured.  I will endeavour to give you as much notice as possible.

  • If an appointment is cancelled I will rearrange a new date and time as soon a possible.

  • If a client is late, the booked appointment time and duration will be adhered to as I cannot make up the missed time by overrunning or adding time to subsequent appointments.

PAYMENT

  • once treatment has been provided payment is due within 24 hours.

  • Payment by cash and bank transfer. (bank details will be given upon request).

  • Gift vouchers are valid for 6 months from date of purchase. 

DATA PROTECTION POLICY

  • New Data Protection Legislation came into force in the UK on 25th May 2018. The General Data Protection Regulation (GDPR), is an EU wide legislation.  Under the 2018 Data Protection Act, I am required to advise my client(s) on my Data Protection Policy.

  • As part of the Client Record,  I am required to retain information for the purpose of consultation for treatment, recording subsequent treatments, and for use by third party medical practitioners or governing bodies/insurers only, at the request of the patient in writing, the insurer or governing body.

  • Upon completion of the Client Consultation Form, Data Protection and consent Form, all paper files, digital images/videos and information therein may be electronically scanned and stored on computer file for as long as the you remain a client of Leigh Chantler Soft Tissue Therapy, and thereafter for a period of 7 years.

  • In the case of minors records will be kept for 7 years after they reach the age of 18.

  • Alternatively, paper records will be retained for the same period. All information provided will be treated as confidential and will not be given to any other person(s)/organisation(s) without the consent of the patient concerned unless required to do so by the governing body or insurer in the case of a complaint or investigation. Information will be held both manually and electronically in files accessible only by Leigh Chantler who is directly involved in the data entry and processing of client records.

  • You have the Individual Rights under the Data Protection act 2018, to be informed about the personal data I hold on you, to access your personal data, to object to the processing of your personal data, restrict the processing of your personal data, to rectify your personal data, to erase your personal data. You can exercise your Individual Rights at any time without charge. However, if your request is considered repetitive, unfounded or excessive a reasonable administration fee can be charged. I will take all appropriate technical and organisational steps to protect the confidentiality, integrity, availability and authenticity of your data.

HEALTH AND SAFETY POLICY

  • The client has a duty to provide accurate and true medical and personal information, to the best of their knowledge.

  • Due to the nature of massage and the need for close contact. Leigh Chantler maintains a high standard of personal hygiene and clients are asked to ensure that they also maintain a high standard of personal hygiene.

  • Leigh Chantler Soft Tissue Therapy is fully insured with Balens Insurance.
